I finally finished this for a friend's baby yesterday:
For the life of me, I can't track down the pattern; it was free and online and gave the pattern for 3, 6 and 9 month sizes. I modified it by adding the blue band to the bottom to match the (handmade!) bias ties. The fabric is an older Amy Butler print.
